If you have been cared for by someone really special at Derriford Hospital, the Royal Eye Infirmary, the Child Development Centre, or a community midwife, the trust wants you to nominate them for an award.
The trust is running the awards scheme to acknowledge the efforts of those staff who go the extra mile for patients, their families and their colleagues.
And as part of the scheme the trust has devised The Herald Readers' Choice award to recognise an individual or team who, as judged by their patients, has delivered excellent care and an excellent service.
Other categories are Leader of the year, Clinical Team of the Year, Non-clinical Team of the Year, Unsung Hero,Volunteer of the Year, Role Model Living the Values, and Excellence in Innovation and Research.
Nominations in all eight categories are being welcomed from members of staff, service users, family, carers or members of the public.
The deadline for nominations is today.
